The problem could be in email system configuration. Please try to test configuration at aMember CP -> Setup -> Email -> Test e-mail sending method. Do you receive test message?

Ok. Enable mail log in amember CP -> Setup -> Email and try to signup again. Then check was message really sent in amember Cp -> Logs -> Mail log. In regards to offline payment, you should use offline payment plugin, in plugin configuration you will be able to modify page with instructions.
